Application for Approval of course of training in Radiation Oncology

To be completed by those who currently hold an accredited training position within one month of commencing an accredited training position.

 

Student Corresponding Membership Form

To be completed by those who do not currently hold an accredited training position but wish to maintain an association with the College for educational purposes.

 

Old Curriculum Examination application forms

 

Part I Examination

To be completed by those seeking admission to the Part I examination. Only original application forms are acceptable and your application must be accompanied by

• payment of the prescribed fee
• a copy of your current Annual Medical Registration Card or Practising Certificate
• 2 passport photos

Part II Examination

To be completed by those seeking admission to the Part II examination. Only current and original application forms are acceptable and your application must be accompanied by:

• payment of the prescribed fee; and
• a copy of your current Annual Medical Registration Card or Practising Certificate
• Logbook
• 2 passport photos

Phase 1 Examination
To be completed by those seeking admission to the Phase 1 examination. Only current and original application forms are acceptable and your application must be accompanied by:

• payment of the prescribed fee; and
• a copy of your current Annual Medical Registration Card or Practising Certificate

Phase 2 Examination
To be completed by those seeking admission to the Phase 1 examination. Only current and original application forms are acceptable and your application must be accompanied by:

• payment of the prescribed fee; and
• a copy of your current Annual Medical Registration Card or Practising Certificate
• 2 passport photos

 

Student training forms

 

Final Certification of Training Form

This form should be completed two months prior to the completion of training and must be signed by your Head of Department or Supervisor of Training.

 

Change in Training Location Form

To be completed by those trainees who have changed accredited training locations.

 

Maternity Leave Form

To be completed by those trainees who have returned to an accredited training position following a period of approved maternity leave.
