Application Forms and Fee Schedules
To apply for Stage 3 Accreditation, which will be per practice site, an application form will need to be completed together with payment of a fee. All charges (eg. assessor travel arrangements) will be at cost to the site for the initial assessment together with an hourly rate for time spent in preparing for the assessment, time spent on-site, time preparing the report on assessment and any other follow-up activities.
