Student Corresponding Membership Form

To be completed by those who do not currently hold an accredited training position but wish to maintain an association with the College for educational purposes.

To be completed by those who currently hold an accredited training position within two weeks of commencing an accredited training position.

To be completed by those who currently hold an accredited training position within two weeks of commencing an accredited training position.

Part 1 and Part 2 Examinations

Part 1 Examination Application Form

To be completed by those seeking admission to the Part 1 examination. Only current application forms are acceptable and your application must be accompanied by payment of the prescribed fee and a copy of your current Annual Medical Registration Card or Practising Certificate.

To be eligible to sit the Part 1 examination, candidates must be Student Members of the College and must be in an accredited training position.

Part 2 Examination Application Form

To be completed by those seeking admission to the Part II examination. Only current application forms are acceptable and your application must be accompanied by:

  • payment of the prescribed fee;
  • and a copy of your current Annual Medical Registration Card or Practising Certificate.
  • Heads of Department/Supervisors of training are required to sign each candidate's Part II Examination application form confirming that the candidate has completed the training requirements and required rotations.

 

Student Training Forms

Final Certification of Completion of Training Form

This form should be completed two months prior to the completion of training and must be signed by your Head of Department or Supervisor of Training.

Please note that you will only be eligible to apply for admission to Fellowship once your signed "Final Certification of Completion of Training" and Interventional Radiology logbook have been received by the College.

Change in Training Location Form

To be completed by those trainees who have changed accredited training locations.

Break In Training Form

To be completed by those trainees who are taking a period of time off from their training program.

Change in Training Status Form

To be completed by those trainees who are commencing part-time training or returning to full-time training.

The College endorses the principle of part-time training. The trainee is required to seek approval from their head of department and all arrangements should be approved at the department level before seeking approval from the Chief Censor. Should part-time training be undertaken, a minimum of 50 per cent full-time equivalent is encouraged.

Maternity Leave Form

To be completed by those trainees who are to commence or who have returned to an accredited training position following a period of approved maternity leave. 

The College endorses that maternity leave, paternity leave or extended periods of sick leave and absence from the training program should not be counted towards a registrars' five-year training requirement. The trainee is required to seek approval from their head of department and all arrangements should be approved at the department level before seeking approval from the Chief Censor.

Log Books

Angiography and Interventional Logbook

Candidates are required to perform 100 angiographic and interventional radiological procedures during their five years of training.

Ultrasound Logbook

Candidates are required to perform 50 ultrasound scans in undifferentiated cases in their first year of training. Logbooks should be sent to the College Office upon completion of a candidate’s first year of training.
